NOTE: This fee does not apply if done in conjunction with a CHRC - Livescan transmittal above.
SIDA / AOA LOST OR NON- RETURNED BADGES
$50.00 first occurrence, $100 second occurrence, $500 third occurrence (includes badge re-issue, if required).
NOTE: If badge is found or returned within 60 days of lost notification and payment, a partial refund less the re-issue charge will be done. No refund after 60 days per occurrence.
SIDA / AOA BADGE RE-ISSUES
$10.00 administrative minimum fee (including STA update, new badge, recordkeeping/supplies) (Authority personnel exempt).
